Referrals:
NO REFERRAL IS NECESSARY, but referrals are gladly accepted. We provide written reports to all referring entities.

Payment:
Our doctors participate in most insurance programs, including Medicare & Medicaid. Your insurance will be filed for you, however, co-payments and deductibles are due at the time of service. Reduced fees may be charged to those patients with documented financial hardship. It is the patient's responsibility to provide the required information.

Office Hours:
Office hours are Monday through Thursday 8am until 5pm.
